At NJ Palliative Care Solutions, we believe that palliative care can transform both patient satisfaction and health system performance when it is delivered with intention, compassion, and data-driven coordination. Our mission is to partner with hospitals, health systems, skilled nursing facilities, and home care providers to create sustainable, scalable models of palliative care across the continuum — from hospital to post-acute to home. By integrating goal-concordant care planning, real-time insights, and interdisciplinary collaboration, we help providers deliver high-quality, cost-effective palliative care that puts the patient’s values at the center. Our Approach
We support a vision of “palliative care without walls” — where patients receive high-quality, compassionate care across every setting: hospitals, skilled nursing facilities, long-term care, home health, hospice, and outpatient environments. Our approach combines clinical program design, data-driven decision-making, and collaborative system integration to help health systems deliver consistent, goal-aligned care. We recruit and train interdisciplinary palliative care teams who are not only clinically excellent but deeply committed to supporting each patient’s journey with empathy, dignity, and expertise.

Our Team

Jessica L. Israel, MD
Chief Medical Officer
Dr. Israel is the Chief Medical Officer at NJ Palliative Care Solutions. She is a geriatrician and palliative care physician committed to person-centered care and the dignity of older adults. She is the Executive Vice President and Chief Medical and Innovation Officer at Complete Care, New Jersey’s largest post-acute and long-term care provider. She previously served as the Senior Vice President of Geriatrics and Palliative care at the RWJ Barnabas Health System. A nationally recognized educator, Dr. Israel has taught at Mount Sinai, Hackensack Meridian, Drexel, and Rutgers, and helped launch Mind the Gap, a statewide transitions-of-care program. Her clinical and leadership work spans geriatrics, dementia, and end-of-life care. She has authored academic and public-facing works, including in The New York Times, and serves on several healthcare and aging-focused boards.

Tina (Anita) Basenese, MA, APN, ACHPN
Chief Nursing Officer
Tina Basenese has worked in the hospice and palliative medicine field in numerous capacities since 1999. A 2003 graduate of one of the nation’s first palliative care nurse practitioner programs at NYU, she has developed, directed, and practiced in inpatient and outpatient palliative care programs, led two hospice agencies, and most recently developed and led a palliative medicine and supportive care program within a large oncology/hematology practice, where she also served as a practicing clinician. She previously served as Vice Chair of the Palliative Care Committee for One Oncology and is a founding member of the NJ Palliative Care APN Consortium, where she held various board positions. A board-certified clinician and experienced administrator, Tina is also an author, educator, mentor, and presenter. She is passionate about ensuring patients, families, and healthcare teams understand the benefits of early palliative care—especially as it relates to transitions of care and effective care coordination for those living with serious illness.
